This happens to me on 14.04 using linux-generic-lts-xenial.

$ lspci -nn | egrep VGA
00:02.0 VGA compatible controller [0300]: Intel Corporation 4th Gen Core Processor Integrated Graphics Controller [8086:0416] (rev 06)

$ uname -a
Linux hostname 4.4.0-59-generic #80~14.04.1-Ubuntu SMP Fri Jan 6 18:02:02 UTC 2017 x86_64 x86_64 x86_64 GNU/Linux

Issue is not consistent, every time I suspend and resume, but happens every week. Logging out from X session and back in solves the issue.
