I tried the release you asked me to (12.04.0) and it exhibits the same problem. The VGA output works fine when the monitor is plugged in at boot up, but not if plugged in after booting.

By the way, I didn't state this before, but the VGA output worked fine under Windows, even when plugged in after booting.
