Intel i915, dual monitor does not work after login

Bug Description
I have gone through a lenghty update from 10.10 to 12.04. The computer is a laptop with Intel i915 graphics. It is connected to a TV set via S-Video cable. I want a mirror display setup, which used to work fine in 10.10.
At the login screen, both the internal LCD panel and the TV screen work as expected.
Both outputs are correctly detected in the "Displays" configuration dialog.
After login i get either:
- internal LCD only - if I turn the S-Video "monitor" off in the "Displays" tool
- TV output only - if I select any other configuration (ie. TV only, both on with mirror, both on without mirror)

What would be useful first would the xrandr output for each of the LVDS / TV configurations you try - just to verify that the display manager is doing as commanded. Then a drm.debug=6 (echo 6 > /sys/modules/ drm/parameters/ debug) dmesg of that procedure (i.e. enable debug before starting the display manager; 'dmesg > dmesg.txt' after trying all the combinations).
And of course it goes without saying, please have all updates applied before retesting.
