HDMI is not working on 16.04; 14.04 worked flawlessly

Bug Description

HDMI monitor is not been detected after installing 16.04.
It worked flawlessly on 14.04
I'm using Lubuntu 16.04

Could you please run the following command once from a terminal by ensuring you have the package xdiagnose installed, and that you click the Yes button for attaching additional debugging information:
apport-collect -p xorg 1617807
