Bug Description
To reproduce:
1) Boot up ubuntu with a second USB keyboard connected
2) Make sure you have more than one layout installed
3) Layout switching works OK
4) Disconnect the keyboard
5) Connect it back again after a while
6) Keyboard layout switching stops working when assigned combination is pressed on USB keyboard, and does not work correctly when assigned combination is pressed on main keyboard (displays ?? in tray area)

Thanks for the report.
This appears to be a generic input bug. I changed the report to reflect this.
Since 9.04 there have been many changes in the X server for input, which means that it is possible the bug has been resolved.
Could you please check again with Ubuntu 10.04?
