Intuos4 (medium) appears to only function if it's connected to pc prior to starting ubuntu.

Works for me (Intuos4 L).

Attach /var/log/Xorg.0.log and output of 'xinput list', and if the previous command shows the wacom devices run 'xinput list-props <id of the stylus device>' and add the output here.

The logfile is from when you had the device attached when booting up? Please boot without the device, then plug it in and attach the logfile..

No, it doesn't work from liveusb either (if tablet is not plugged in during boot up). Tested with precise pangolin daily image.
