urfkill seems to get stuck when stressing flight mode

symptoms:
when rapidly turning flight mode on and off, the SIM will show as either "No Sim" or "Offline" after a few iterations

how to reproduce:
open network indicator
(randomly) turn flight mode on/off
repeat
check SIM status when flight mode is off

I was able to get the sim back online by removing /var/lib/urfkill/saved-states

After discussing with Alfonso this morning, your syslog definitely shows signs of the ofono bug mentioned above in comment #6. I asked Alfonso to mark this as Critical/rtm14 as when this bug is hit, the modem becomes unresponsive until the phone is restarted ( technically an ofono restart would work, but a non-technical user would have no idea how to do this ).

Also regarding this bug, in order to determine whether there's an underlying urfkill bug, we need to query the modem states ( via /usr/share/ofono/scripts/list-modems ) and the state of urfkill ( /usr/share/urfkill/scripts/flight-mode & enumerate ).

urfkill is designed to only toggle Flight-Mode if it's able to reliably enable/disable *all* the radios. If any of the online/offline operations fail, the flight-mode operation fails, and urfkill will rollback any radios that had been changed up to the failure.

Also how easily can you reproduce?
