The issue seems to be /etc/apt/sources.list.d/prequists-sources.list which in it has:

# sources.list fragment for pre-requists (mirror from sources.list + fallback)
# this is safe to remove after the upgrade
deb http://ubuntu.osuosl.org/ubuntu/ precise-updates main

# when this changes from -proposed to -updates, ensure to update
# DistUpgradeController.py:1393 as well
deb http://archive.ubuntu.com/ubuntu/ precise-updates main

Changed in update-manager (Ubuntu):
status: New → Triaged
importance: Undecided → Medium

It looks like there is no facility to remove the PreReq file.

I got the same error after upgrading with some ppa repositories after enabling them again in the software sources manager. Now it won't really let me do anything and tells me to report it:

"'W:Duplicate sources.list entry http://ppa.launchpad.net/pjbroad/ppa/ubuntu/ precise/main amd64 Packages (/var/lib/apt/lists/ppa.launchpad.net_pjbroad_ppa_ubuntu_dists_precise_main_binary-amd64_Packages), W:Duplicate sources.list entry http://ppa.launchpad.net/pjbroad/ppa/ubuntu/ precise/main i386 Packages (/var/lib/apt/lists/ppa.launchpad.net_pjbroad_ppa_ubuntu_dists_precise_main_binary-i386_Packages), E:Encountered a section with no Package: header, E:Problem with MergeList /var/lib/apt/lists/packages.ubuntu.com_precise_unknown-horizons_dists_release_main_binary-amd64_Packages, E:Pakkelisterne eller statusfilen kunne ikke tolkes eller åbnes.'"

The pjbroad archive is related to the game Eternal Lands. Unknown Horizons is another game. Last line says "The package lists or the status file could not be interpreted or opened".
