lucid -> precise main all upgrade: system upgrades but upgraded system is still lucid

Bug Description

Lucid to Precise upgrade main all i386/amd64

Lucid upgrades but not completely and the resulting system shows that it is still Lucid (lsb_release, kernel, version of python)
The upgrade finish without error.

Logs are attached as artefacts to the result of the jenkins job.

bug about apt-term.log that shows only packages being removed is bug 940240

I disabled the code that captures debconf prompts - comment the line DebconfLog from profile/default.cfg.d/default.cfg - and this error is not triggered.

I didn't try yet but we should be able to trigger this bug with a manual upgrade of main using the following command:
$ DEBIAN_FRONTEND=editor EDITOR="cat>/tmp/debconf.log" do-release-upgrade -d
