swap partition lost during the upgrade to 11.4

Bug Description
During the upgrade from 10.10 to 11.4 swap partition was lost, because devices were renamed.
In /etc/fstab was
/dev/mapper/
instead of
/dev/mapper/
I'm running Ubuntu on dual boot machine together with Windows 7. Windows 7 uses additional partition hosting D: disk. Ubuntu and its swap space were in separate primary partitions. There was some empty space between D: disk and swap space.
I can't provide more information about partitions at the time of the upgrade, because I've rearranged them in the meantime.
