Failed upgrade from 11.10 (Oneiric) to 12.04 (Precise)

Bug Description

Trying to upgrade a server from 11.10 to 12.04 and it failed.

It seems that libc6 cannot upgrade because of two reasons:
- an old version of libc-bin (2.13-20ubuntu5.3) is required but is no longer in the repos. I have 2.15-0ubuntu10.3.
- a newer version of the linux kernel (2.6.24 or later) is required. I have 2.6.18.

But I'm uploading the logs anyway cuz I'm still not clear on what I need to do to fix it.

1:0.152.25.13

I've come across lots of other stories of people having trouble making this upgrade, and I also read that the only way to skip this upgrade is to start with the previous LTS release, 10.04. So I'm installing that version now and will attempt to make the jump from there.
