It currently happened with gedit and Firefox. I will check back here if I find other apps behaving like this. Please, do tell if I can provide further assistance.

Just checked with Terminal. If I open Terminal with Ctrl+Alt+T, it launches the application with its window "glued" to the upper left corner of the screen and it doesn't show up in App switcher. If I drag the window and put it elsewhere, it then appears in the App switcher.
However, if I open the Terminal app from the application list in Dash home, it appears in the App switcher right away and it launches the window in the middle of the screen.

Dunno if this behavior is because of opening the app from a key combination (that shouldn't be the case) vs opening it from Dash home, or because of the initial positioning of the application window.

LE: Scratch that. Now it works from anywhere I open it (it shows up in App switcher). What the hell is going on ?! Something's really messed up.

Please update you system and make sure you have unity 5.4.0 or above and let us know if the issue still is there.

Yes, of course it's the latest version, 5.4.0. Was other version bundled with 12.04 ?
