Alt-tab switcher: behavior when window closes while switching

When one of the windows in the switcher closes while switching, the corresponding item stays in the launcher and when selected opens a new instance of the program. I'd expect the item to get removed or at least not start an instance of the closed program.

TEST CASE:
1. Open a gnome-terminal.
2. Run "sleep 10 ; exit"
3. Initiate the alt-tab switcher and wait for the terminal to close.

RESULT:
* The terminal item doesn't get removed from the switcher.
* Selecting that item results in launching a new gnome-terminal.

If this is by design, feel free to close as "Opinion".

Using Unity trunk r1455, not sure if that's a Compiz issue instead.

When the program cannot be restarted (may it be a python program or the program was removed in the meantime), this results in a crash.

Possibly related... if a window appears while you are in the alt-tab switcher, an icon does not appear.
