window title doesn't disappear after closing window/application

Binary package hint: unity

After closing an application or window, the title doesn't disappear from the top bar across the screen. If the window is maximized/fullscreen, then the window buttons (close/minimise/maximise) also stay after the window is closed. The title and window buttons disappear after clicking on the desktop. This can be replicated every time.

1. Start firefox by clicking on button in the launcher that appears on the left of screen.
2. Close firefox by clicking on the red [x].
3. Firefox window title (and optionally buttons if it was fullscreen) still show up in the top bar.
4. Click anywhere on the Desktop. The title and buttons disappear.

I can reproduce the bug as well. The focus is not restored to the desktop after closing a window. However when closing a window on a desktop with multiply windows, the focus is restored to another window as expected.
