Unity has an issue where menus don't appear

Unity has two issues that are both fixed by running unity --reset in a tty.

Issue one is that the menus don't appear please see attached video
Issue two is that the screensaver doesn't blank the screen so it looks like the system has frozen completely.

Upon hitting ctrl-alt-f1 and running unity --reset everything works as expected. The screen is blanked and the password to unlock is made available and the menus then appear, there are then no more issues until you reboot/restart the machine again.
