Windows don't open in correct position

Bug Description
Using a laptop with 2nd monitor plugged into HDMI port for dual monitor setup. 2nd monitor is directly above laptop monitor in Displays tool and Mirror *is not* enabled.
Two issues:
1: Regardless whether launcher icon is clicked on top or bottom display the app always opens in the top display.
2. When the app opens in the top display it opens as if the Unity Launcher and menu bar are not there. In other words it opens aligned with the top left corner of the screen. The window's title bar is hidden under the menu bar and the window's left border is hidden under the Unity launcher.
My settings for Appearance are to show menus in the window's title bar and the launcher auto hide is set to off.
These settings have been like this "forever" it is my preference. Windows used to open on the display on which the launcher icon was clicked and aligned with the top left corner of the junction of the launcher and menu bar.
This new behavior began about a month ago. Don't remember the exact date. Just finally got tired of ignoring it and decided to report the bug.

Top half of image is top monitor.
File browser left margin and top window border hidden by Unity Launcher and menu bar respectively.
File browser was launched by clicking the Files icon in the Unity Launcher on the bottom screen.
