Launcher - sometimes when app icons are dragged and dropped from the Dash to the Launcher the icon disappears

Bug Description
Sometimes when a application is dragged and dropped from the Dash to the Launcher the app gets successfully added to the Launcher, but the application icon disappears to that the app is represented in the Launcher as a empty space between the other app icons! See attached screencast invisible_
