Window title should not be set from page title

Bug Description
Currently, the window title is set from the title of the current page. There are several problems with this:
1) The page title appears directly below the window title, resulting in duplication of information. There's no need to have both right on top of each other.
2) The page title and window title serve rather different purposes. The page title should let you know where you are within an app. The window title should help you locate that app amongst all the other windows. As such, it should feature the application name and perhaps the current document, not the current location in the application.
3) It is difficult to set the window title when the header is hidden, by setting the page title to "". (See bug #1308343.)
Instead, the SDK should provide a way to set the default window title for all pages, as well as override it on a per-page basis.
