"Check disc for defects"(integrity-check) does not work

A built image with ubuntu-defaults-builder cannot run "Check disc for defects" on gfxboot.

How to reproduce:
 1. build a raring image with ubuntu-defaults-builder
 2. boot with the built image
 3. select "Check disc for defects" on gfxboot

Expected result:
 show result of integrity-check

Actual result:
 return to gfxboot immediately

ubuntu-defaults-builder and live-build do not generate /cdrom/md5sum.txt which casper expects

We are in the similar situation as:

We can generate /cdrom/MD5SUMS with LB_CHECKSUMS=md5 in ubuntu-defaults-builder, but cannot rename it to "md5sum.txt", because binary hook in ubuntu-defaults-builder is executed before "lb binary_checksums" is called.

We can fix this in live-build or casper, not ubuntu-defaults-builder, I think.
