Thanks for your report. This is also bug 48732, and occurs due to strange model names on certain disks. This is fixed in 6.06.1, so you can work around it by upgrading all packages with 'ubiquity' in their names on the running live CD before starting the installer.

When you say that certain disks have strange model names, do you mean the
disk labels? What are the model names that you are referring to? Would it
help if I rename the disk labels to something simpler?

Hello,

From other guys' feedback, model names means the model of particular makers
or model number; However my case is different, that was some minor
inconsistence which can be fixed easily by Windows OS utitlity, and those
errors caused the installer crashed.

I listed model name of the disk I used by then.
