Installer Crash after partitioning.

I have an AMD64 Sempron system - Install crashed. Kubuntu will install but had problem with errors after. I also have a windows xp partition on the same system. I partitioned 30GB for Ubuntu and 2 GB for the linux swap.Here is the debug code for the install crash:
