Installer crashes right after disk partitioning and formatting

Bug Description

The "forward" button in installation summary screen was inactive first time I defined partitions' size. When I returned to custom partitioning screen with "back" and went to summary again, "forward" button was functional. After formatting, installer failed with following message:

Thanks for your report. This is also bug 43012. Perhaps you could follow the instructions I gave down at the end of that bug for producing more information that I need to fix it, and post them to that bug? Thanks in advance.

Sorry for filing the duplicate. I did do a search but didn't find 43012.

I have given away the hardware that this bug showed up on. It was
unavoidable. I wish I could keep working on it, but I don't have access
to that machine any more.

I did install Dapper on it, but used the installer CD instead of the
Live CD.

One thing that I didn't think about until after the fact is that I was
not adding any swap partition. The computer has 768MB of RAM and I had
run Breezy for months with no swap, so when I started the Dapper
install, I simply didn't create a swap partition. Maybe this
contributed to the problem?
