Your install media came back as corrupt:
"CasperMD5CheckResult: fail"

You either had a broken ISO download, or wrote to faulty media. You should verify your download[1] and try different media if it passes.

Further to Further to comment #2 and no response from reporter after over a year I'm closing this as "Invalid".
