Default ext4 (the default) this happens twice I have since installed successfully the same is on the same hardware. It was very odd behaviour and I am at a loss as to how you would replicate it. I think it to be a rare case and cannot offer any more info as to what caused it. Perhaps it shall reappear with other others.
