Bug Description
I was installing Ubuntu 16.04 LTS from a USB flash disk, everything was fine until I was at the stage of choosing where to install to because when I chose the partitioned part of my drive I wanted to install on the installation crashed.

At one point it appears that the installer created a partition to install Ubuntu to ( /dev/sda6 ) but then it vanished. Did you open another disk management utility and delete it?
