system crashes when ubuntu installing

Bug Description

I want to install Ubuntu on my new ssd disk.
but ten times I got a error the same error.

How I divided my disk:

sda1 swap ~8500
sda5 ext4 ~ 1tb

sdb1 / ext4 ~50000
sdb5 /home ext4 ~~70000

Your installation medium is corrupt; you will need to recreate it and may want to verify it using the check disc option at the boot menu.
