UpgradeStatus: No upgrade log present (probably fresh install)

I am trying to install from a usb drive because there is no cd / dvd drive. I have tried installing both the desktop and the server editions and all efforts result in a failed installation.
I have also booted to the live edition (on the USB), and tried installing from that. The results again result in a failure.
I have tried reformatting and repartitioning the hard disk, and repeated the installation process with the same results.
If I try the custom install, I can´t get past the "No Boot ... identified."

Thank you for taking the time to report this bug and helping to make Ubuntu better. Reviewing your log files attached to this bug report it seems that there is a problem with your installation media (CD/DVD). You can verify the integrity of the Ubuntu ISO files you downloaded by following the instructions at https://help.ubuntu.com/community/HowToMD5SUM. You might also retry your installation with new media. In the event that is is not in fact an error with your installation media please set the bug's status back to New. Thanks and good luck!

The response does not help, as I am not installing from a CD/DVD. it is from a USB. I have tried both the ISO installation throughunetbootin-windows, and creating a live version on the usb and then installing it. This resulted in a message that the system was properly installed, but when I reboot, I get nothing but errors and the system doesn´t boot up.
