It looks like there may be an issue with the media from which you were installing:

Sep 29 08:01:20 ubuntu-gnome apt-setup: You may try the --cdrom option to set the CD-ROM mount point. See 'man apt-cdrom' for more information about the CD-ROM auto-detection and mount point.

Could you try checking it for defects? https://help.ubuntu.com/community/Installation/CDIntegrityCheck

Hi Brian,
I was able to get this working, by following the instructions on the following bug report:
https://bugs.launchpad.net/ubuntu/+source/ubiquity/+bug/658865

Essentially all I needed to do was the following:
sudo rm /usr/lib/ubiquity/apt-setup/generators/40cdrom

And it got past the point of the installation that kept crashing.
I wasn't actually installing via a CD, I was using a usb live-cd created in linux mint using unetbootin.

Feel free to close this if you would like (or do i need to? This is the first time i've submitted a bug) :)

Cheers,
Alistair.
