Also seeing the following issue with indicator-network:
- turn SIM pin on in the system settings
- reboot the phone
- Now the network indicator always shows the SIM as "unknown" and it never comes online, so it can't be used or unlocked.

Reverting to the telepathy-ofono package from https://launchpad.net/ubuntu/utopic/armhf/telepathy-ofono/0.2+14.10.20141007-0ubuntu1 fixes the issue for me, and I can again send SMS and make calls.

Under RTM 5 on Mako, I'm having the same bug, and I managed to PUK lock my phone. Found another work-around:

Firstly, I had not set a sim pin, and settings showed this wasn't locked. After trying to 'unlock' my pin three times unsucessfully, I was asked for my PUK code. SIM was screwed, basically. Was able to search for the PUK code for Telstra, and found a site where I could enter my details and retrieve it. If you are with Telstra in Australia, you can find this under My Account.

Once entering this, I found I could then choose a pin. Now each time I reboot I need to enter this pin and all is fine. I tried unsetting this however, and got the same problem again - sim was locked while saying it was not, etc. Was only able to resolve it by entering the WRONG pin three times, reentering my PUK code, then setting my pin again.
Bug seems to be that pin lock needs to stay on for now. Anyway, have downgraded ofono package on my device in case I need to downgrade in a hurry. Looking forward to this being fixed.

per dave:
mako isn't recognising that the sim is locked, I get unknown in the indicator, no sim pin in the boot, and no unlock option in the indicator, I can however go into settings, security, simpin and unlock it then then it works.

@Pat, this one:

https://github.com/rilmodem/ofono/pull/129

which is merged, but the plan was to land that in vivid, as this does not affect Krillin. Do we want this for RTM?

I would prefer that the ofono task in this bug be removed, as it's essentially a duplicate of bug #1376250, which as Alfonso pointed out has already been committed to trunk. I added an rtm14 tag and an Ubuntu (RTM) task to that bug so that it can be escalated and landed in one of the OTA buckets.

I changed the ofono (Ubuntu) task to FixReleased, as it's believed that the fix for bug #1376250 also fixes this problem. This fix landed in vivid as ofono 1.12.bzr6882+15.04.20141126-0ubuntu1.

As this same fix is also slated for ww51-2014, I've updated the ofono / Ubuntu (RTM) task to FixCommitted due to the active merge proposal for RTM:
