It seems your modem will work. keywords I saw in your modemdata.txt say it's an slmodem that uses the snd_intel8x0m module.

see this link to make it work: https://help.ubuntu.com/community/DialupModemHowto#head-4be470bbae4b120bd5f6101c9aa218df7518b0c7
section name is Modems supported by ALSA drivers (snd_atiixp_modem, snd_via82xx_modem, snd_intel8x0m), follow instructions until "Modems supported by the Smartlink driver"

driver does not give any hints as to how the user can make it work (using the alsa intel module and the sl-modem-daemon). I would mark this confirmed (because user's modem doesn't work out of the box, and same happened to me with another alsa-enabled modem) but I'm aware sl-modem-daemon has some license issue and hence in multiverse.

For your modem to work out of the box, the sl-modem-daemon should be in the installation CD of Ubuntu afaik.

They will most probably close this bug bc the sl-modem-daemon (the daemon that makes your modem work) cannot be in the Ubuntu CD due to license issue (daemon has distribution restrictions I believe).

Hopefully the bug won't be closed and sl-modem will get an implementation instead that enables it to suggest to the user what to do.

The slmodemd in the Ubuntu repository seems to be broken for ALSA support.
For example:

 sudo sl-modem-daemon-2.9.10+2.9.9d+e-pre2/usr/sbin/slmodemd -c USA --alsa modem:1
SmartLink Soft Modem: version 2.9.9e-pre1 Sep 30 2007 00:07:03
symbolic link `/dev/ttySL0' -> `/dev/pts/1' created.
modem `modem:1' created. TTY is `/dev/pts/1'
error: getpwnam Slmodemd: Success
---------

However:
ls -l /dev/ttySL0 /dev/pts/*
crw--w---- 1 marv tty 136, 0 2007-10-06 09:07 /dev/pts/0
crw--w---- 1 marv tty 136, 2 2007-10-06 08:50 /dev/pts/2
lrwxrwxrwx 1 root root 10 2007-10-06 09:06 /dev/ttySL0 -> /dev/pts/1
so the reported port /dev/pts/1 was in fact NOT created

This can be contrasted with the slmodemd from http://linmodems.technion.ac.il/packages/smartlink/
within the tarball SLMODEMD.gcc4.1.tar.gz
$ sudo slmodemd -c USA --alsa modem:1
SmartLink Soft Modem: version 2.9.11 Aug 6 2007 22:02:27
symbolic link `/dev/ttySL0' -> `/dev/pts/1' created.
modem `modem:1' created. TTY is `/dev/pts/1'
Use `/dev/ttySL0' as modem device, Ctrl+C for termination.

Opening another console and checking:
$ ls -l /dev/ttySL0 /dev/pts/*
crw--w---- 1 marv tty 136, 0 2007-10-06 09:12 /dev/pts/0
crw-rw---- 1 root uucp 136, 1 2007-10-06 09:12 /dev/pts/1
crw--w---- 1 marv tty 136, 2 2007-10-06 09:14 /dev/pts/2
lrwxrwxrwx 1 root root 10 2007-10-06 09:12 /dev/ttySL0 -> /dev/pts/1
port creation is confirmed.

On Waleed' system the command would be that appropriate to HDA card hosted modems
$ sudo slmodemd -c USA --alsa hw:0,1
or
$ sudo slmodemd -c USA --alsa hw:0,6
as constrasted to my AC'97 type using modem:1

Note that to provide ALSA support for slmodemd (as I did for the SLMODEMD* tarballs,
the slmodemd MUST be compiled within the modem/ subfolder with command
$ make SUPPORT_ALSA=1

   If so then sl-modem-daemon should work. If not then I'm afraid
   it's not supported (by the drivers we provide in gutsy, anyway).

 * Does this problem still occur with the sl-modem-daemon from
   current gutsy ?

towsonu2003:

  sl-modem-daemon is supposed to be on the CD but you have to install
  it. The restricted-manager system is supposed to offer it to you.

  sl-modem-daemon (just that binary package, and not the unsupported
  sl-modem-source package) is in restricted now, not multiverse.

  Your comment is completely unrelated to this bug report. Please
  file a separate bug (which I will then discuss there).
