signon-ui keeps spawning facebook "security warning page" in background

Bug Description
For the last few days, I've noticed a page suddenly appear in my browser on the facebook domain (happens every few hours). The page is on a simple white background with the following text:
Success
SECURITY WARNING: Please treat the URL above as you would your password and do not share it with anyone.
The URL looks like this: https:/
Last time, it happened at the same time that signon-ui told me that I'd been logged out of google so I thought, maybe signon-ui is renewing its login on all my accounts and that's somehow triggering the facebook page.
To verify that, I opened signon-ui and went to the facebook account. As soon as I did, it spawned a new copy of the blank page and it did that each time I tried to log in via signon-ui.
I tried revoking access to ubuntu in my facebook settings in the hope that that would clear any bad state. After doing that, when I try to log in via signon-ui it spawns the facebook confirmation page in the browser instead of in signon-ui's in-window browser and when I click through to the end then I get the security warning page.
