Can't choose options for a new project in "Define Project Details"

Bug Description
When I create a new project (Qt Quick Controls Application), they are 2 options at the step "Define Project Details" :
- With ui.qml file
- Enable native styling. Requires dependency of the QtWidgets module.
These 2 checkboxes can't be selected... I can click on it but the case still stay unchecked.
I use Qt Creator 4.0.2 on Ubuntu 16.10 without any ppa. My packages are updated every time as needed.
