xcb_use_themed_cursor.patch dropped incorrectly with Qt 5.4.2

Bug Description

Unlike the other XCB patch, this patch was not actually included yet in the Qt 5.4.2 release. It should be restored.

It causes a cursor issue in webbrowser.

The patch originally fixed bug #1277020 "hideous mouse cursor when hovering over hyperlinks.", and is included in current vivid since March (https://launchpad.net/ubuntu/+source/qtbase-opensource-src/5.4.1+dfsg-2ubuntu4)
