This would have happened somewhere along the path of:
 * Connect to bluetooth headset
 * Open sound prefs to select that headset on input & output
 * Open Google Plus hangout for video chat

From the stack trace, it looks like there are non-complete frames coming in over bluetooth, i e the last bytes do not add up to a full sample.
I'm not very familiar with the bluetooth code in PulseAudio so I'm unsure of how to fix this in the best way.

I just saw (I think) this bug, similar recreate.
the result was that I had no audio devices available after the crash.
Ie, in the sound settings there was nothing.

Thank you for reporting this bug to Ubuntu.
Ubuntu 12.04 (precise) reached end-of-life on April 28, 2017.
