Logitech USB headsets work as output devices only after once restarting pulseaudio

Bug Description

Binary package hint: pulseaudio

Using Lucid, my 2 USB headsets are displayed under "Hardware" and can be configured (e.g. Analog Stereo Output + Analog Mono Input). But they are not shown under "Output". On the other hand, my bluetooth headset is shown under Output and can be selected as the default sound device.

When I followed the directions at https://wiki.ubuntu.com/PulseAudio/Log, the headsets started working, and they have been working since, with several reboots.

Is the procedure (starting it manually) required for the usb audio device to function as output?

Same for me, headet's shown under "Hardware", but not under "Output" or "Input". When I kill pulseaudio and start it again manually (either as shown under https://wiki.ubuntu.com/PulseAudio/Log or by just typing "pulseaudio" or "nohup pulseaudio &") then the headset is also shown under "Output" and "Input" and works properly.

I can confirm it happening (lucid testing, lucid stable and maverick) my USB sound device based on C-Media CM106 chipset (the device is called "USB 7.1ch sound card", has both analog and optical I/O). Restarting PA helps until reboot.

I was able to reproduce this on a newly installed Maverick AMD64 system with a "Logitech Premium Stereo USB Headset 350". I plugged in the headset after booting the system, and it showed up in the hardware tab of gnome-volume-control, but not on the input or output tabs.

If I tried to use the test button on the hardware tab, gnome-volume-control would crash with the following printed to stderr:

    **
    ERROR:gvc-speaker-test.c:390:get_channel_map_for_card: assertion failed: (stream)
    Aborted

I was able to play audio to the headset with aplay by sending it directly to the appropriate plughw device instead of to pulse.

After restarting pulseaudio, the headset worked as expected.

My i686 Maverick installation demonstrates this issue. In my case, I am using a Plantronics CS60 USB Headset. Force killing any running instance of the pulseaudio daemon followed by a manual restart of the pulseaudio service fixes the issue.

I've experienced the same problem off and on with my USB Logitech ClearChat Pro against Maverick, Natty and now Oneiric. I'm able to work around the problem so that it's basically a non-issue by using `pulseaudio -k`. Have we determined whether this is actually a problem with PulseAudio or if it's a problem with something else?

Ubuntu 10.04 (lucid) reached end-of-life on May 9, 2013.
