Do you hear any login sound at all, and have you tried killing pulseaudio and restarting it?

To kill and restart pulseaudio, do the following:
$ pulseaudio -k
$ pulseaudio -D --log-target=syslog

Login sound works.

After running for some time, and every time after a suspend and resume, ALSA and pulseaudio sound no longer works. OSS works.

Killing and restarting pulseaudio fixes this.
