Bug Description
How to reproduce:
1. Start Rhythmbox / Totem
2. Rapidly Start and Stop music/video
What is expected to happen:
The audio (flash-player, Totem and Rhythmbox) should output their sound.
What actually happens:
If music/sound is played I hear something like "fast forwarded" bells (high pitched randomness)
Pulseaudio -vvvvv:

How to work around (get sound working again):
1. Shut off all audio-sources.
2. Unplug headphones
3. Mute sounds
4. Unmute sounds
5. Plug in headphones
6. Mute and Unmute sounds
When did I register this issue the first time (I don't know if it regressed at this exact date but in 12.04 this bug never happened):
