Bluetooth headset shows up in sound settings only on initial setup, but won't appear on subsequent connects

Bug Description

When connecting to a BeeWi bluetooth receiver, which identifies itself as a bluetooth headset, ubuntu successfully pairs with it and it shows up in the sound settings as an audio output device. But this works only once. Subsequent attempts to connect with the device are also successful but it won't be listet as an audio output device anymore.

I tried removing the device from the paired devices list and re-pairing but it still does not show up.

Ubuntu 12.10 (quantal) reached end-of-life on May 16, 2014.

We appreciate that this bug may be old and you might not be interested in discussing it any more. But if you are then please upgrade to the latest Ubuntu version and re-test.
