sound card not detected by pulseaudio

Bug Description

I have a pandora PX5D usb sound device which works perfectly with ubuntu most of the time.

Sometimes it fails to show up as an available sound input/output in the sound indicator, but alsa will still recognize the device and aplay will let me send audio to it without problems.

Ive found a way to consistently reproduce it:

start the device
the device is detected
select the device as sound output
start playing music (tried mplayer and rhythmbox)
turn off the device
turn on the device
the device is no longer detected
pulseaudio -k
pulseaudio --start
the device is again detected.

Ubuntu 12.10 (quantal) reached end-of-life on May 16, 2014.
