3) What is expected to happen is when one opens https://bugs.launchpad.net/ubuntu/+source/evince/+bug/531516/+attachment/1207516/+files/test.pdf one may click one of the four buttons, and then clicks another, the first one disappears, and reappears in the secondly clicked button, just like Adobe Reader.

What happens instead is clicking the second radio button does input a mark in the second button, but the first button clicked still has its mark. This was originally reported to be a regression.

WORKAROUND: Use Google Chrome's built-in PDF viewer.

Confirmed.

Also, same on Windows as well, running Evince 2.30.3: Created a fillable PDF in OpenOffice 3.2.1. Form displays and operates properly using Acrobat Reader 9. Radio buttons do not function in Evince.

Also, Evince seems to be incapable of even properly displaying other fillable forms, such as those available here:
http://live.gnome.org/Evince/Forms

Thank you for taking the time to report this bug and helping to make Ubuntu better. The issue that you reported is one that should be reproducible with the live environment of the Desktop CD of the development release - Oneiric Ocelot. It would help us greatly if you could test with it so we can work on getting it fixed in the next release of Ubuntu. This same bug (or at least what looks like this same bug) is still present as of
Evince 3.2.1p1 running on OpenBSD 5.1-stable amd64, using the twm window manager.

My test pdf file is US Treasury Form 90-22.1 (revised 1-2012), available from
  http://www.irs.gov/pub/irs-pdf/f90221.pdf

If (after downloading that file, which is a fillable PDF form), I type "evince f90221.pdf"
then I can find no way to select one of the radioboxes for item 2. In particular, clicking
on or around any of the options has no visible affect on these radioboxes, nor does typing
any character I can think of.

'evince --version' reports
  GNOME Document Viewer 3.2.1

The form works fine using Acrobat 9.5.1 Pro Extended under Windows 7 Ultimate SP1.
