After some hours of googling, I finally found the culprit: The PageSize policy setting.

But setting it to 1 didn't work for me (Brother HL-5150D). Simply removing the "/Policies ... /PageSize ..." line from the prolog in the testpage-a4-pdftops.ps testcase causes the printout to be correct.
