Bug Description
The root file system (ext4) had been corrupted, but it was not checked and repaired during boot. I had to run GParted from a live session to check and repair the file system.
I used to see a file system check in Plymouth splash (under the Ubuntu logo and dots) during boot in earlier releases. I think it stopped working now.
