"form design" and "form controls" toolbars visible in writer

Bug Description

After opening a form in oo-base "form design" and "form controls" toolbars are
enabled in other components of OO as well. When I start a brand new text
(writer) document, I get these toolbars too.

Dapper, upgraded as of March, 12:
1. Open base document.
2. Choose "Create form project"
3. Close form.
4. Open new writer document.

The toolbars are still there...

It doesn't matter whether I quit openoffice between steps 3 and 4.
