all language-support-extra-* packages are universe. As language-selector really checks for language-support-extra-*, I have filed the wishlist bug 363619 to remove hunspell-de-med from language-support-extra-de (which would make it obsolete) as temporary workaround. I guess a properly working ooo spell checker will make more users happy than an additional medical dictionary. ;)
