Creación fallida de una nueva base de datos

Tengo un grave error al crear una nueva base de datos en el servidor tengo el siguiente error:

Trato de localizar el archivo en el cual esta el error pero no lo encuentro, de igual forma pienso que es una actualización del servidor en Ubuntu, porque el mismo servidor me coje en otra máquina que no esta actualizado el ubuntu. Gracias

I have unmarked it as a security issue since this bug does not show evidence of allowing attackers to cross privilege boundaries nor directly cause loss of data/privacy.
