Not able to connect to the Internet

Bug Description
I previously used to use Ubuntu 10.10.
Whenever I was not able to connect to the Internet, it used to show a window which asks for username & password for DSL authentication.
Now I am using Ubuntu 11.04.
Now if the above scenario occurs, the same window gets popped up but it does not ask for any authentication. So I am not able to connect to the Internet.
There are only 2 buttons - "Cancel" & "OK" which is disabled.
I have attached screenshot of the problem for refence.
