The PUK unlock dialog in nm-applet only accepts four digits

Bug Description
Binary package hint: network-
If you have a PUK-locked sim card you'll get a popup from nm-applet asking to enter the PUK-code. Unfortunately the input field only accepts four digits and since PUK-codes are usually eight digits long it is not possible to enter the correct code. Also, the label for the entry field says "PIN:" when it should be "PUK:" (see attachment). The input probably shouldn't be in clear text either.
