network-manager applet does not show wifi interface

Bug Description

Per the 10/3/2016 post for bug 1574347, I'm submitting my own bug for no wifi interface and the wrong icon on the network-manager applet on a Thinkpad T440p running 16.04 LTS. Even though I see the wifi disconnected icon and no wifi networks in the applet drop down, I am still connected to a network and can access the internet.

It seems to run like this for a while, including several wake/sleep cycles but then eventually won't connect at all and then I end up restarting.
