Unity Icon will not launch MySQL Workbench after upgrade to Xenial

Bug Description
1. Upgraded from 15.10 to 16.04
2. Click MySQL Workbench - has "slow fade" in and out for a few seconds. MySQL WorkBench fails to launch.
3. Launch Terminal. Type mysql-workbench and enter to launch - this is successful.
4. Now there are two icons in the Unity launcher bar.
# Fix
5. Unlock old icon from launcher and lock new icon to launcher (if it didn't already lock automatically)
I think this is a bug and whatever has changed in /usr/share/
