I think that the root cause is a corrupted p2m_host[] list via a PV-GRUB bug. Updated PV-GRUB AKIs are now available. These can be used in us-east-1 to verify the fix:

32-bit: aki-805ea7e9
64-bit: aki-825ea7eb
